Death
I can only find three documents mentioning his death: this Facebook post from the music library of one of his alma maters, UNT (the only one listing a date); this brief blog post; and this blog post mentioning his widow. None have references, and in fact the only link in the first is to his WP article. His profile at UNT doesn't mention his death, and I can't find a traditional obituary anywhere.
