The Palliative Father: The Living Legacy of Rev. Fr. Joy Koothur

In a world where healthcare is often treated as a commodity, one man dared to envision a sanctuary where healing comes without a price tag. Meet Rev. Fr. Joy Koothur, the Co Founder, CEO, and Trustee of Shanthibhavan Palliative Hospitals. Affectionately known across the nation as "The Palliative Father," Fr. Joy has earned this title through a lifetime of absolute surrender to the service of the bedridden and marginalized. By stepping away from conventional pastoral duties, he turned personal empathy into a revolutionary public mission: pioneering India’s very first "No-Bill" hospital model. Born on December 9, 1974, in the serene village of Venkitangu, Thrissur, Kerala, Joy was raised in a loving family by his parents, K.V. Thomas and Mary Thomas, alongside his siblings Lilly, Varghese, and Antony. Even as a young boy, Joy was a natural leader, fiercely protective of the weak and inherently generous. However, his true calling crystallized during his late teens. He witnessed a destitute man selflessly caring for an isolated, bedridden elder. This profound display of raw humanity, combined with the timeless charitable legacy of Mother Teresa, lit a fire in his soul that would define his destiny. Answering the call to priesthood, he joined St. Mary’s Minor Seminary in Thrissur (1990) and later earned a Degree in Philosophy from St. Joseph’s Pontifical Major Seminary, Carmelgiri. He was ordained on December 28, 2000. For over a decade, Fr. Joy served the Archdiocese of Thrissur. He wasn't just a priest behind a pulpit; he was an action-oriented, people-centered shepherd. Known for his fearless defense of the poor, he possessed a rare humility that could de-escalate the most volatile parish conflicts and restore harmony. Yet, the silent cries of the bedridden and suffering continually tugged at his heart. Driven by a divine restlessness, he approached Archbishop Mar Andrews Thazhath with a radical request: to completely dedicate his life, health, and energy to serving the poorest of the poor through full-time, free palliative care. In December 2016, that vision materialized. In collaboration with the Franciscan Sisters of St. Clare, co-founded alongside V. Rev. Sr. Maria Chiara Kalliyathuparambil FSC and the late Sr. Beatrice Scalinci FSC, Shanthibhavan Palliative Hospital was born. Shanthibhavan broke the mould of modern medicine. It stands proud as India’s first long-term palliative hospital where absolutely everything is free. From admission, accommodation, specialist doctors, and 24 hour nursing care to complex procedures like dialysis, fluid tapping for cirrhosis, echocardiograms, ultrasound scans, and continuous oxygen therapy, patients face a bill of exactly zero rupees. What began as a single sanctuary has grown into a powerful network, with free hospitals operating in Pallissery (Thrissur) and Vattappara (Thiruvananthapuram), alongside specialized medical centers across Kombodinjamakkal, Sakthan Nagar, Aranattukara, Wadakkanchery, and Aluva. Fr. Joy is globally recognized as the pioneer who introduced the concept of Long Term Palliative Hospital Care for bedridden patients to the world stage. To take this compassionate framework beyond Indian borders, he established Shanthibhavan Palliative International, which now boasts an active presence in Australia, Italy, and the United States. His ultimate goal is to set a new global standard for humanitarian healthcare.

But Fr. Joy’s compassion is also incredibly smart. Recognizing the gaps in elder care, he integrated modern technology into his mission: JOY’S TOUCH - A revolutionary smart health watch that monitors the 24/7 vitals of bedridden or elderly individuals living entirely alone. Fr. Joy’s heart beats for every facet of human suffering. His tailored humanitarian projects include: Devadeyam Elder Village, a beautifully designed "Elder Village" featuring assisted living facilities where the elderly can live out their golden years with pride, joy, and dignity. Early Medical & Cancer Detection Project, a proactive screening initiative designed to catch terminal illnesses early in children and young adults. Health Caravan, free mobile medical camps traversing the rural landscapes of Thrissur and Ernakulam. Oottuvandy, when inflation and rising LPG prices hit local kitchens hard, Fr. Joy launched this mobile kitchen project to deliver nutritious, non profit, and free meals directly to the doorsteps of isolated elders living alone. Fr. Joy’s compassion is backed by immense courage. During the devastating 2018 Kerala Floods, he didn't watch from the sidelines. Maneuvering a jeep through treacherous waters, he personally led rescue operations to save vulnerable children and the elderly. He converted Shanthibhavan into an active relief camp and mobilized a 50 member squad to disinfect and rebuild flood ravaged homes. Fr. Joy’s relentless spirit has caught the eyes of the world. He was honoured with the prestigious International Humanitarian Award, nominated by the UNESCO affiliated International Institute for Peace through Tourism (IIPP) in Geneva. Representatives from 81 nations voted unanimously to bestow this honor upon him. In a moving tribute, the IIPP committee described Fr. Joy as a man of "boundless compassion" and officially wrote to Pope Francis to highlight his exemplary, Christ like work. Closer to home, he was also honored with a special award by the Vallachira Panchayat for his heroic flood relief efforts. Today, Fr. Joy Koothur remains a man on a mission. Unmoved by fame and fuelled entirely by love, he continues to push boundaries with a singular, beautiful dream to establish a free, world class palliative hospital in every single district of Kerala, ensuring that no human being ever has to face pain, isolation, or death alone.
