The story follows Reacher, a former major in the United States Army Military Police Corps, who has agreed to work with the US Drug Enforcement Administration (DEA) to bring down suspected drug smuggler Zachary Beck by going undercover as the bodyguard of Beck's son, Richard. During the investigation, Reacher encounters an adversary from his military past, and must balance his desire for revenge against his need to finish his mission.
In October 2024, ahead of the third season premiere, it was confirmed that a fourth season was in production. It was later revealed that filming for the fourth season would begin in the summer of 2025.

In Maine, Reacher is selling some vinyl records when he saves a young man from a kidnapping attempt. The driver of the young man's car is knocked out and the kidnapper is killed by Reacher before he can get to the passenger. When Reacher also accidentally shoots a cop as well, a police chase ensues. Reacher manages to outrun the police as the young man, Richard Beck, reveals to Reacher that someone had kidnapped him before and that his father, Zachary Beck, only paid the ransom when his ear was amputated. Richard takes Reacher home to see his father. Reacher meets with Zachary Beck, who thanks him for saving his son but is suspicious of him. Zachary offers Reacher a job if he plays Russian roulette, which he does. Turns out Reacher was working undercover for DEA agent Susan Duffy the whole time. Duffy recruits Reacher to find a missing undercover agent, Teresa Daniels, in exchange for helping Reacher deal with a previous nemesis, Lieutenant Colonel Francis Xavier Quinn, whom he saw exiting a car owned by Zachary. To put Reacher undercover, they staged Richard's entire kidnapping, with the kidnapper and the "shot" cop being DEA agents Steven Eliot and Guillermo Villanueva, who are working with Duffy. In the middle of the night Reacher sneaks out of his room and into the Beck estate's garage, where he finds a hidden underground bunker and reveals to Duffy that evidence of Teresa was there.

When Duffy learns from Reacher that Teresa is in danger, she beats up Richard's driver and bodyguard, whom they are holding captive, to find out what they plan to do with Teresa, but the bodyguard claims he does not know. Reacher has a brief scuffle with hulking bodyguard Paulie before Zachary's second in command, Chapman Duke, sends his henchman Angel Doll and Reacher to check out the hijacked cars from the kidnapping. Reacher calls Duffy to tell her that there may be a problem in the kidnapping story; Reacher told them he had killed the bodyguard with a grenade, and so they would expect a car covered in the bodyguard's blood. Duffy goes to the car yard to retrieve one of the cars. Another car is in the way, so Duffy won't be able to move the vehicle. Doll notices the bodyguard's car but Duffy changed the license plate and Reacher plays it off as one of the thousands just like it. Duke orders Reacher to take a truck to New London, Connecticut. Reacher calls Neagley to investigate the people he works with. Reacher meets with Duffy to check the trailer for drugs or Teresa's body, but they only find carpets as it's a loyalty test. Back in Maine, Doll questions Reacher about his actions at the yard and in the kidnapping, and when Doll tries to tell Duke about his suspicions, Reacher kills him.

After returning from New London, Reacher meets with Zachary over a voicemail from Doll. Doll ran the plates on the truck that was 'trying to kidnap' Richard and they belonged to some partners Zachary had done "rug business" with previously. Zachary was told no one who worked there matched the description of the kidnapper, nor do they use Uzis. Reacher convinces him the kidnapper was simply a new hire trying to send a message. Zachary is perplexed because those previous partners have also disappeared. Reacher tries to make him believe that his partners are trying to betray him and that they should kill them, but Zachary doesn't listen. After this, Reacher calls Neagley and learns Zachary's crew is full of thugs and ex-cons. Reacher then phones Duffy to have a car and change of clothes, preparing to go back to the warehouse where he killed Doll. Reacher swims to the meeting point. At the warehouse, Reacher and Duffy search for Teresa and clean up Doll's remains; two men arrive, and Reacher and Duffy kill them. Duffy keeps Doll's laptop and uses it to email Zachary, leading his men to a hideout. The next day, Reacher takes Richard into town so he can buy a beloved pop cap gun for his father's birthday. Reacher defends him from some bullies, and Richard tells what's really going on. Zachary Beck created a bribe-filled shipping racket to import rugs from overseas. Julius McCabe wanted to use his rugs to smuggle illegal shipments, Zachary said no, so Julius kidnapped Richard and cut his ear off. Julius is now "Zachary's boss." When Reacher returns home, he goes with Duke and Zachary to a cabin, supposedly the kidnapper's hideout (in reality, a DEA safehouse). Reacher takes the opportunity to kill Duke, fake a shootout and blow up the hideout; in the car, Reacher shows he "found" Doll's gun (that Reacher had the whole time) and recognized two of the men in the hideout as the same he killed in the warehouse. This convinces Zachary that he was betrayed by all three and Reacher becomes his number two in command.

Reacher heads to Duffy's house after gaining Zachary's trust. He confirms from photos the agents took that Julius McCabe is in fact Xavier Quinn, whom Reacher believed he had killed. Following the revelation, Reacher tells Duffy the story of Dominique Kohl, an investigator assigned to Reacher's 110th Special Investigator team. After a few cases under their belt, Reacher and Kohl investigate a case involving Roland Gorowski selling state secrets. They eventually force Gorowski to confess that he works for Quinn because Quinn kidnapped his little girl and threatened his family. Reacher and Kohl, along with Private Anthony "Tony" Frasconi, intercept one of Quinn's buyers, a Syrian businessman, Safwan Qasim, whom they force to assist in capturing Quinn. Having enough evidence to prosecute him, Reacher wants Kohl to get the acclaim and sends Kohl and Frasconi to arrest Quinn. After hearing nothing for over an hour, Reacher assesses something has gone wrong. Marines storm Quinn's house to find Frasconi killed with Kohl and Quinn missing. Reacher tracks down Quinn but it's too late for Kohl, Quinn tortured her to death. Reacher finds Quinn and leads him to a cliff, where he shoots him in the head and Quinn falls into the rough waters. Reacher later deduces he was using Quinn's pistol and the bullet likely didn't pierce his skull, leaving McCabe/Quinn alive. Back in the present, Zachary tells Reacher they found Doll's laptop GPS signal at one of Zachary's facilities, the same one that Duffy and her partner are snooping around in. Zachary explains three of Quinn's men are already on their way. Reacher heads to "assist" Quinn's men and can't get a hold of Duffy or her partner. Quinn's men arrive as the credits roll.

When Reacher can't contact Duffy, he calls Eliot in the cabin to call the facility's number. Duffy realizes someone calling while the business is closed is unusual, answering just as Eliot tells her what's happening and a gunfight breaks out between Quinn's men and Duffy and Villanueva. Reacher helps Duffy and Villanueva escape, leaving one of Quinn's men alive to bury suspicion. At the cabin, the captive bodyguard manages to kill Eliot and escape. After discovering that the feds are spying on them, Quinn's men move into the house. When Duffy finds Eliot dead and the bodyguard has escaped, she calls Reacher to tell him so and to leave the house, but Reacher disagrees and formulates a plan involving Villanueva. Duffy cuts the power for half the city, giving Reacher a reason to watch the front gate, where he would undoubtedly run into the bodyguard. Reacher turns over Villanueva's car in the middle of the road and uses Villanueva inside as bait to kill the bodyguard. Back at the house, Zachary and Quinn's men tear the house apart, surely about to find Duffy's badge that Reacher picked up off the floor of the warehouse shootout from earlier. However, Zachary surprises Reacher when informing him that Annette, the maid, was an ATF infiltrator and that Paulie accidentally killed her while interrogating her. Reacher tells Duffy that several Feds are spying on Quinn's operation and that Quinn is dealing in firearms, not drugs. The following morning, Zachary informs Reacher that they've been summoned by Quinn to meet with them. Reacher tells Duffy who stakes out the place with Villanueva. Zachary and Reacher arrive at Quinn's factory full of guns as credits roll.

Quinn meets with Zachary and berates him for unknowingly having an ATF mole in Zachary's house. Due to the retrograde amnesia caused by Reacher, Quinn has no memory of him and introduces himself to him using his alias McCabe. Someone passes Reacher with a green dress which they later learn is meant for Teresa who is still alive. Quinn meets with a Russian mobster; Reacher discovers Quinn is in danger as it was the Russian mob that gave Quinn money to expand his operation. Quinn can't pay up as a very large arms deal is being held up by bad weather at sea. Afterwards, Reacher drives into town with Richard to meet with Duffy and return her badge; Reacher and Duffy kiss, and when Richard sees Villanueva come around the corner he realizes that his attempted kidnapping was a setup and Reacher is undercover. Quinn sends men to kill everyone investigating him, including an active member of the 110th and Neagley; Neagley kills one and maims another, getting him to talk. Neagley tells Reacher what she knows, but it's too late for the 110th soldier. Quinn arrives at Zachary's house to question him about the U.S. Army investigating him; Zachary tells him that said unit was Reacher's. Quinn sends Paulie to kill Reacher, but he escapes, leading to a chase. Reacher kills all the men in the forest except Paulie and takes refuge in a laundromat. Quinn punishes Zachary by forcing Richard to play Russian roulette before having Paulie cut off Zachary's ear. Tracing down a number attained from the tailor of the green dress, Reacher and Duffy search for Teresa in Port Rome and find an isolated boat. They swim out to search it and find evidence Teresa was there. Harley, a member of Zachary's security team, returns to the boat to find Reacher and Duffy. After being bested, Harley claims that Quinn changed everything after Reacher escaped; they leave Harley to die on the burning boat as it explodes. Reacher and Duffy head to Los Angeles.

Neagley interrogates Costopoulos, the man who hired her attackers, who tells her that Quinn ordered the hit. Costopolis also explains Quinn murdered a family when business went wrong and is about to do the same to the Beck family. Reacher and Duffy, in Los Angeles, track down Darien Prado, a drug dealer working for Zachary who Duffy previously tried to bust before going to Reacher. Later that night Duffy and Reacher have sex. Reacher and Duffy intercept Darien at a club and blackmail him into cooperating. Darien says he'll call Zachary and arrange a meetup, as long as Reacher and Duffy don't bust him for his third felony (putting him in jail for life). Reacher and Duffy promptly betray him and Darien is busted as they leave. Neagley is at the meetup when Zachary arrives. Neagley explains to Zachary what Quinn does when family businesses go wrong and that he and Richard are in danger. Quinn visits a captive Teresa and continues drugging her. Reacher and the team discover that the weapons are being sold to Yemeni buyers for terrorist acts to be carried out in the United States. Zachary finds the toy gun Richard bought him and apologizes to Richard for his wrongdoing and says he will protect him at all costs. Zachary informs Reacher of the exchange happening at a shipyard after Quinn tells him. Duffy and Villanueva involve the ATF, overruling Reacher who storms out. The ATF are pissed Duffy and crew did so much wthout informing them, allowing Duffy, Reacher, and Villanueva to observe only. The ATF sends dozens of agents to the shipyard. Leaving the observation van, Reacher puts together his sniper rifle he secretly purchased after storming off, dead set on killing Quinn despite Duffy's dedication to putting Teresa first. As he is about to kill Quinn and become a federal fugitive he calls Neagley to erase all ties to him. While talking to Neagley Reacher realizes the exchange is actually taking place at Zachary's house, and the place they are at is a setup.

When Reacher kills Quinn's men, Quinn kills the buyers and takes their money and Richard hostage. Quinn is confronted by Zachary, who points the toy gun at him; when the broken barrel glued together melts, Quinn kills Zachary and leaves without the money. Quinn is confronted outside the house by the Russian gangster Taktarov and his men. Reacher and Neagley offer Taktarov the money in exchange for Quinn, and they accept. Reacher reminds Quinn about Kohl before executing him.
