There are three aspects to your question.

a) Literally, only in a few cases. Most articles do not have the pending changes lock.

b) Practically, it depends on the article. The more active editors there are on the page, and/or the stronger the consensus is, the likelier it is that you should first discuss on the Talk page out of prudence. In general, though, the process followed is "bold-revert-discuss". Just make sure you do not edit war.
