The back of the book blurbs are not independent sources. Those are solicited by publishers and authors and are uniformly positive. For the award, there are many sources online, such as this and this. They may not be better than what you have, but including one would give readers an online source. I'll add the ISBN and date to the book. Reviews of the book are still needed, not just the information about the award for the poem.
